(EOS), a condition defined by the presence of progressive spinal deformity of various etiologies in the growing child, can rapidly develop small, stiff, distorted thoraces Current methods (growing rods and VEPTR) for management of early onset scoliosis associate with kyphosis have not been satisfactory. 4 rib construct is a surgical choice that is used in Palestine for more than 8 years now We have experience with good results management of EOS with this technique for patients with hyperthoracic kyphosis , thoracolumbar kyphosis , severe scoliosis and myelomeningocele Advantages 1- Easy application 2- Strong anchor proximally 3- Avoidance of kyphogenic effect secondary to rod lengthening 4- Adjustment of the rod to control the apex with the cantilever effect 5- Long lever arm without fusion 6- No need for post operative brace with the dual rod Conclusion 4 rib construct can be an alternative choice in developing countries No massive bleeding No dangerous procedures follow-up problems are not life threatening With this complex and demanding medical condition in developing countries , we think that 4 rib construct may be a good and safe surgical option .
