Radical Cystectomy in Female Patients with Invasive Bladder Tumor

Purpose: The incidence of invasive bladder tumor in female patients is less than that of male and the behavior of these tumor in females is not well investigated(in our country)compared with that of males.

Material and methods: Out of 432 patients who underwent radical cystectomy,31 cases were female patients. The mean age of these patients was 63.32+7.96. The types of the tumor, grades, lymph node involvements and survivals were investigated. In terms of these parameters comparison between male and female cases were done.

Results: Urothelial cancer(UC) were detected in %67 and %79 in female and male patients respectively. The peri-operative complication rate in female were %6.5. The stages of the tumors were; pT2(%41.4), pT3(%44.8), pT 4(%13.8). The percentage of the lymph node involvements and grade III of the disease were %44.4 and % 82.8 respectively.  Tumor stages, grades and histologic types were indifferent from that of male patients(p>0.05). However lymph node involvement were higher in female patients(p=0.019). Disease specific free survival in female and male patients were 20.37+2.5 and 67.8+3.95 months respectively(p=0.014). In multivariate analysis age and sex were found to have no  significant effect on survival rates p=0.22, p=0.25 respectively. Tumor stages, histologic types  and lymph node involvements were the significant factors which effected disease free survival (p=0.008, p=0.026, p=0.017 respectively, Cox regression test).

Conclusion: Survival rates in female patents is less than that of male patients. We believe that the high  incidence of lymph node involvements may justify this findings. Therefore early detection of the tumor is recommended.
