Endoscopic management of upper and lower ureter stone using pneumatic lithotripsy: Tips and tricks

PURPOSE: To evaluate and compare the success rate of semirigid ureterorenoscope in the management of upper and lower ureter stones using pneumatic lithotripsy(PL)
MATERIALS-METHODS: Ninety- two patients with a mean age of (45 ± 15years) (male 73.9%, female 26.1%), who were operated by a single experienced surgeon between 2013-January and 2015-July were included. A lot of tips and tricks were used especially in the management of upper ureter stone to improve the success rate and avoid stone migration. The mean stone size was 8.8 ± 2.6 mm. Forty-two patients (45.7%) had upper ureter stones, and fifty patients (45.3%) had lower ureter stones. The medical records were retrospectively reviewed in terms of (age, sex, stone size, stone location, success rate, complications, and presence of hydronephrosis). The success rate was defined as a complete extraction of the stone as a unit or its fragments. 
RESULTS: Eighty-four patients became stone free (91%). The success rates for patients with upper or lower ureteric stones were 93% and 90%, respectively (p=0. 63). There were no intra operative complications. Only 4 patients (4.4%) had postoperative complications in term of urinary tract infection or urosepsis, and they were managed with appropriate medical therapy. The success rate showed no correlation with age, gender, stone volume, stone location or presence of hydronephrosis. 
CONCLUSION: Semirigid ureterorenoscopy using PL is a safe and a feasible treatment option for the treatment of upper and lower ureter stones. Experienced surgeon utilizing tips and tricks of ureterorenoscopy does improve the success rate, especially in upper ureter stones.
